Are you an employee of a company participating in the LI Wellness Challenge? You have come to the right place! 

Please fill out the following registration form.  When you click "NEXT" you will be taken to the American Heart Association page where you will see a blue box that says "Register with Start!"  Click "Register" and you will gain access to a all of the Individual Online Programs that will help you set goals, track your progress, and reach those goals!
